Theoretical basis,composition and application evaluation of chronic care model
Abstract：Chronic care model(CCM)was a comprehensive and system management method for chronic disea-ses.It analyzed the theoretical basis,the content of the elements,the application evaluation,and the feasibility of CCM in China.It was proposed that we should learn from the CCM theory frame,combined with our national conditions,improve the existing management model,so as to improve the management efficiency of chronic dis-eases.
