Abstract：It introduced the development,implementation process and features of “Patient Centered Medical Home”model and further to probe into the role of nursing personnel in this model.It put forward that nurses must master more general medical knowledge and information technology,and be good at communicating,coor-dinating and training leadership,so as to promote health care reform and promote the development of discipline.
