Research progress on restraining factors of“hospital community family”chronic disease continuity of care
Abstract：Chronic disease is a major threat to human health .The hospital community family three in one continuity of care is effective means to reduce the incidence of chronic diseases and improve the quality of life of patients .From the policy factors ,the service provider factors and service recipient factors ,it reviewed the re‐straining factors of developing chronic diseases continue service ,in order to provide references for the policy makers and service providers .
