Influence of transitional care model on pulmonary function and quality of life of discharged patients with COPD
Abstract：Objective:To probe into the application effect of transitional care model(TCM)for pulmonary func-tion and quality of life of discharged patients with COPD.Methods:A total of 108 patients with stable COPD discharged from hospital were randomly divided into control group and observation group,54 cases in each.The traditional care model was implemented for patients in control group,and the patients in observation group re-ceived TCM.The pulmonary function and quality of life of two groups of patients were compared before and af-ter the intervention.Results:There was statistically significant difference in pulmonary function and quality of life of patients between both groups before and after the intervention(P <0.05).Conclusion:Transitional care model has provided continuity of care services for COPD discharged patients and improved patients’pulmonary rehabilitation behavior,thus to improve lung function and enhance the quality of life of patients.
