Appl ication of continuity of care mode for senile patients with chronic disease

Abstract：Objective:To probe into the effect of continuity of care mode applied for senile patients with chronic disease.Methods:A total of 800 patients were randomly divided into observation group with 412 cases and control group with 388 cases.Patients in two groups were given routine nursing,besides that the continuity of care was provided for patients in observation group.Results:The proportion of reasonable diet,good life habit,accurate medication and regular referral and satisfaction degree af-ter discharge in observation group was significantly higher than that in control group(P <0.05 ),the number of re hospitalization was signifi-cantly less than that in control group(P <0.05 ).Conclusion:According to the different rehabilitation stages of patients,the targeted continuity of care plan is formulated,so as to improve patients’compliance to treat-ment and medication and enhance their quality of life and satisfaction to medical care services.
