Application research on clinical pathway in chronic diseases management of strengthening community hypertension patients

Abstract：Obj ective:To explore an effective mode of community nurses par-ticipated chronic disease management for patients with hypertension.Meth-ods:To choose confirmedly diagnosed primary hypertension of moderate risk layer patients in the same street of two residents committee as research sub-j ect.Among them,those meet the inclusion criteria were divided into two groups.Patients in control group adopted the conventional management method.And patients in observation group were given chronic disease man-agement for patients with hypertension based on the clinical pathway(nurse version,doctor version,and patient version)worked out according to the con-tents of “Chinese hypertension prevention and treatment guideline”.Then up to the standard rate of hypertensive related knowledge,blood pressure con-trol rate and management satisfaction of two group cases were observed.Re-sults:There were statistical significant differences in terms of up to the standard rate of hypertension knowledge of health education,blood pressure control rate and management satisfaction between the two groups(P<0.05). Conclusion To apply clinical pathways in management of hypertension chron-ic disease in community can enhance up to the standard rate of health educa-tion knowledge and control the rate of blood pressure of hypertension pa-tients.And meanwhile it can raise the j ob satisfaction of community nursing and performance ability of community nurses.
