Comparative analysis of the clinical efficacy of InterTan intramedullary nails and PFNA internal fixation in the treatment of osteoporotic intertrochanteric fracture in elderly

Abstract：Objective To compare the efficacy of InterTan intramedullary nail with proximal femoral nail antirotation(PFNA)internal fixation in elderly patients with osteoporotic intertrochanteric fractures(ITF)of the femur.Methods 70 elderly patients with osteoporotic ITF admitted to our hospital from June 2023 to August 2024 were selected as the research subjects.They were divided into an experimental group(treated with InterTan intramedullary nail fixation)and a control group(treated with PFNA fixation)using a random number table method,with 35 cases in each group.The perioperative indicators,complications,fracture healing time,Harris scores at 1 month and 6 months after surgery,excellent and good rate of hip joint function recovery at 6 months after surgery,as well as refracture and internal fixation loosening within 12 months after surgery were compared between the two groups of patients.Results The operation time,blood loss,postoperative blood transfusion volume and hospitalization cost of the experimental group were greater than those of the control group(P<0.05),while there were no differences in hospital stay and the incidence of complications between the two groups(P>0.05).The full weight-bearing activity time and fracture healing time of the experimental group were shorter than those of the control group(P<0.05),and the Harris scores at 1 month and 6 months after the operation were higher than those of the control group(P<0.05).The excellent and good rate of the experimental group was higher than that of the control group(P<0.05).There were no differences in the incidences of refracture and internal fixation loosening between the two groups(P>0.05).Conclusion Both InterTan intramedullary nail and PFNA internal fixation have good effects in the treatment of senile osteoporotic ITF.Although InterTan intramedullary nail increases the operation time,intraoperative blood loss and hospitalization expenses,it can significantly shorten the postoperative full weight-bearing time of patients and promote the recovery of joint function.
