Effect of cisatracurium combined with etomidate on anesthesia efficacy and perioperative adverse events in patients with open reduction and internal fixation

Abstract:Objective To explore the effect of cisatracurium combined with etomidate on anesthesia efficacy and perioperative adverse events in patients with open reduction and internal fixation.Methods A total of 86 patients scheduled for open reduction and internal fixation at Peking University First Hospital Taiyuan Hospital from January 2024 to January 2025 were selected as the research subjects and randomly divided into a control group and a study group,with 43 cases in each group,using a random number table.All patients underwent open reduction and internal fixation.The control group was anesthetized with rocuronium combined with etomidate,while the study group was anesthetized with cisatracurium combined with etomidate.The anesthesia efficacy(anesthesia onset time,anesthesia maintenance time,anesthesia depth,degree of muscle relaxation),the surgical incision healing indicators(healing grade,healing days),the postoperative recovery situation(recovery time from anesthesia,extubation time,time of the first out-of-bed activity,time of exhaust,defecation time),and the perioperative adverse events(incision infection,redness and swelling,exudation,nausea and vomiting)were compared between the two groups.Results There were no statistically significant differences between the two groups in anesthetic depth,muscle relaxation degree,healing grade,bone healing days,time to first ambulation,exhaust/defecation time,or overall incidence of perioperative adverse reactions(P>0.05).The study group had longer anesthetic onset time[(3.25±0.56)min vs.(2.18±0.32)min],anesthetic maintenance time[(80.52±12.45)min vs.(65.48±10.23)min],awakening time[(30.56±5.23)min vs.(25.18±4.15)min]and extubation time[(45.68±8.12)min vs.(38.25±7.05)min]compared with the control group(P<0.05).The incidence of postoperative residual muscle relaxation was lower in the study group[5 cases(11.63%)vs.13 cases(30.23%)](P<0.05).These differences were statistically significant.Conclusion In open reduction and internal fixation,compared with rocuronium,the use of cisatracurium combined with etomidate can prolong anesthetic onset time,maintenance time,awakening time and extubation time,but significantly reduce the incidence of postoperative residual muscle relaxation without increasing other perioperative adverse reactions or affecting healing and intestinal function recovery.No significant effects on bone healing and bowel function are observed.
