Abstract:Objective: To explore the causes of early anastomotic stenosis and anastomotic leakage in patients with gastric cancer after surgery, to enhance clinical physicians' understanding of anastomotic stenosis and anastomotic leakage, and to provide references for the diagnosis and prevention of anastomotic complications. Methods: A case of a patient with gastric cancer treated at No. 940 Hospital of the Joint Logistics Support Force, Chinese People's Liberation Army in December 2024 was reviewed, and the diagnostic and therapeutic process and prognosis of early anastomotic stenosis and anastomotic leakage were analyzed. Results: The patient underwent thoracoscopic and laparoscopic combined radical surgery for gastric cancer and cervical gastroesophageal anastomosis. On postoperative day 3, the patient developed symptoms such as dysphagia and fever. CT examination showed exudation around the anastomosis. Infection was not effectively controlled with active treatment. After attempting to eat, the patient experienced nausea and vomiting, and gradually developed redness, swelling, and odor at the neck incision. Gastroscopy confirmed that the cause was suture penetration leading to anastomotic stenosis and anastomotic leak. The sutures were removed, and nutritional support was provided via a jejunal feeding tube. The infection was gradually controlled, and the patient resumed oral intake on postoperative day 25 and was discharged smoothly. Conclusion: There are multiple causes for anastomotic stenosis and anastomotic leakage after gastric cancer surgery. It is particularly important to broaden thinking, choose appropriate adjuvant treatments, surgical timing, and refine surgical techniques to prevent postoperative complications.
