Application of intracoronary shockwave balloon combined with plaque rotational atherectomy in PCI for severe coronary artery calcification

Abstract:Objective To explore the application effect of intracoronary shockwave balloon combined with plaque rotational atherectomy in percutaneous coronary intervention(PCI)for severe coronary artery calcification.Methods Sixty patients with severe coronary artery calcification who underwent PCI in Pizhou Hospital of Traditional Chinese Medicine from March 2023 to January 2025 were selected as the research subjects.They were divided into the observation group and the control group,with 30 cases in each group.The control group was pretreated with plaque rotational atherectomy during the operation,while the observation group was pretreated with intracoronary shockwave balloon combined with plaque rotational atherectomy during the operation.The perioperative conditions,the incidence of intraoperative complications and the incidence of major adverse cardiac events(MACE)after the operation,the incidence of restenosis and vascular hyperplasia were compared between the two groups.Results There were no statistically significant differences in operation time,intraoperative stent implantation quantity and diameter between the two groups(P>0.05).The minimum lumen diameter and area after dilation in the observation group were larger than those in the control group(P<0.05).There were no statistically significant differences in intraoperative complications between the two groups(P>0.05).There were no statistically significant differences in the incidences of MACE at 30 days and 6 months after surgery between the two groups(P>0.05).There was no statistically significant difference in the total incidence of restenosis and vascular hyperplasia at 6 months after surgery between the two groups(P>0.05).Conclusion Intracoronary shockwave balloon combined with rotational atherectomy for lesion preparation in patients with severe coronary artery calcification undergoing PCI can achieve larger minimal lumen diameter and area without increasing the rate of complications.
