Effect evaluation of precise tension-reducing sutures in plastic surgery on neck incision scarring in patients undergoing open thyroid surgery

Abstract:Objective To investigate the impact of employing precise tension-reducing suturing technique from plastic surgery for incision closure during open thyroid surgery on scar formation.Methods A total of 164 patients who underwent open thyroid surgery through neck approach in the First People's Hospital of Lianyungang from August 2022 to August 2023 were selected and randomly divided into observation group and control group,with 82 patients in each group.The observation group underwent trapezoid-shaped skin flap trimming combined with heart-shaped subcuticular suturing,whereas the control group received standard subcuticular continuous suturing technique without skin flap trimming.The following parameters were systematically compared between the two groups:incision length,suturing time,post-operative pain Visual Analogue Scale(VAS)scores,flap edema severity,scar width,and Vancouver Scar Scale(VSS)scores.Patient satisfaction levels were additionally analyzed to evaluate inter-group differences.Results No statistically significant inter-group differences were observed in operative parameters including incision length,suture duration,or surgical flap edema incidence(P>0.05).The pain score of the observation group was lower than that of the control group on the first day after surgery(P<0.05),and there was no significant difference between the two groups on the third day after surgery(P>0.05).Six months after surgery,the scar recovery in the observation group was better than that in the control group,and the scar width was(2.85±0.15)mm,which was narrower than that in the control group of(4.62±0.18)mm.95.1%(78/82)of patients in the observation group had no scar hyperplasia or depression,the VSS scar index score was lower,and the patient satisfaction rate was 95.1%(78/82).The results were statistically different(P<0.05).Conclusion The application of precise tension-reducing suturing technique from plastic surgery for thyroidectomy wound closure has demonstrated significant efficacy in minimizing scar formation,optimizing postoperative aesthetic outcomes,and enhancing patient satisfaction,thereby meriting widespread clinical adoption.
