Technical key points and clinical application of laparoscopic radical resection for hilar cholangiocarcinoma:with high-definition surgical video

Abstract:Laparoscopic radical resection of hilar cholangiocarcinoma(HCCA)has significant advantages over traditional open abdominal surgery.However,due to the intricate anatomy of the hilar region,numerous technical challenges are associated with this surgical approach.This article presents a complete laparoscopic radical resection of hilar cholangiocarcinoma to comprehensively analyze its key technical aspects and evaluate its clinical application value.Pre-operative assessment is performed using enhanced computed tomography(CT),magnetic resonance imaging(MRI)and three-dimensional reconstruction to determine the extent of tumor involvement according to the Bismuth classification.The surgical video mainly demonstrates the key points of lymph node dissection,lowering of the hepatic hilar plate,"tunnel method"dissection of blood vessels,and micro-anastomosis.The"book-turning style"exposure method during the operation effectively overcomes the limitations of the surgical field.Research indicates that laparoscopic radical resection of hilar cholangiocarcinoma is comparable to open abdominal surgery in terms of the number of retrieved lymph nodes and patient survival prognosis,while also featuring less intraoperative blood loss and a shorter hospital stay.The successful implementation of this surgical approach depends on accurate pre-operative evaluation,the surgeon's excellent skills,and standardized teamwork.The surgical video records relevant techniques and experiences,providing a visual teaching example for this technology.
