Analysis of the effect of laparoscopic cholecystectomy in treating acute cholecystitis complicated with impacted stones in the cystic duct

Abstract：Objective To analyze the effect of laparoscopic cholecystectomy in the treatment of acute cholecystitis with impacted stones in the cystic duct.Methods 156 patients with acute cholecystitis complicated with impacted stones in the cystic duct admitted to Yixing Second People's Hospital from January 2019 to December 2023 were selected and divided into the control group(7 cases,open cholecystectomy)and the observation group(78 cases,laparoscopic cholecystectomy)by the method of ball drawing.Observation indicators for statistical analysis included indicators related to intraoperative situation conditions(surgical time,incision length,blood loss),indicators related to postoperative recovery(the first exhaust time,fasting time,time of getting out of bed,hospitalization time),pain indicators(VAS score),inflammatory response indicators(CRP and IL-6 levels),and complication occurrence indicators(total occurrence rate).Results The indicators related to general surgical conditions and postoperative recovery in the observation group were better than those in the control group,with P values all＜0.05.In addition,the VAS score,CRP and IL-6 levels,and complication occurrence rate in the observation group were significantly lower than those in the control group,with P values all＜0.05.After months of follow-up,the incidence of biliary stricture and the recurrence rate of stones in the observation group were lower than those in the control group,and the differences were statistically significant(Pvalues all＜0.05).Conclusion Laparoscopic cholecystectomy has certain advantages in terms of less trauma,less bleeding,faster recovery,less pain,milder inflammatory response and lower complication risk,and can achieve good results in the treatment of acute cholecystitis complicated with impacted stones in the cystic duct.
