Comparative analysis of the efficacy of hollow nails versus Endobutton in the treatment of Lisfranc ligament injuries

Abstract:Objective To evaluate the clinical efficacy of Endobutton versus traditional hollow compression screw fixation for the treatment of Lisfranc ligament injuries through a comparative analysis.Methods A retrospective analysis,spanning from February 2020 to September 2024,was meticulously carried out on thirty-four patients who were diagnosed with and treated for Lisfranc joint injuries at the First Affiliated Hospital of Baotou Medical College Hospital,providing valuable insights into the management of such injuries.Of the patients,fifteen were treated with Endobutton fixation and nineteen with traditional hollow compression screw fixation.A comparative analysis was conducted to evaluate the clinical efficacy of the two surgical modalities in terms of clinical baseline data,peri-operative parameters,pain,postoperative American Foot and Ankle Society(AOFAS)scores,postoperative foot function score excellence rate,the incidence of postoperative complications,and postoperative plantar pressure assessment.Results A comprehensive series of intricate statistical tests were conducted to thoroughly evaluate the two groups of patients in multiple aspects including age,gender,the time span from injury occurrence to the commencement of surgery,intraoperative haemorrhage and other indicators.The results demonstrated that the above observed differences were not statistically significant(P>0.05).The postoperative visual analogue scores(VAS)of the two groups demonstrated a notable decline,whereas the American Society of Foot and Ankle Surgery scores(AOFAS)exhibited a marked increase,and the differences were statistically significant(P<0.05).At 4,6 and 12 months post-operation,the VAS and AOFAS scores of the observation group exhibited a significantly superior outcome in comparison to the control group,with a notable statistical significance(P<0.05).In the observation group nine patients were evaluated as excellent,four patients were good,two patients were moderate and none of the patients were evaluated as poor.The excellence rate was 86.7%.In comparison,the control group demonstrated a significantly lower degree of overall excellence,with five patients rated as excellent,seven rated as good,four rated as moderate and three rated as poor,resulting in an overall excellence rate of 63.1%.The excellent and good rate of the observation group was significantly higher than that of the control group.In the observation group,a total of two cases of postoperative complications were detected,including one case of incision infection and one case of partial skin necrosis injury.In contrast,a total of six cases of postoperative complications were identified in the control group,one instance of incision infection,two occurrences of internal fixation loosening and breakage,two cases of traumatic arthritis and a single case of nerve injury,and the difference between the two groups was statistically significant(P<0.05).Conclusion The clinical efficacy of Endobutton fixation for repairing ligamentous injuries of Lisfranc joint is superior to that of traditional hollow nail fixation.
