Laparoscopic partial splenectomy using the modified Warshaw method for the treatment of severe traumatic splenic rupture

Abstract:The operation was performed under general anesthesia with endotracheal intubation.A urinary catheter and nasogastric tube were placed preoperatively.The patient was placed in the right lateral position.An observation hole was made 2~4 cm above the left side of the umbilicus to explore the abdominal cavity and recover autologous blood.When opening the gastrocolic ligament,more omental vessels and short gastric vessels were preserved.The main trunk of the splenic artery was separated above the tail of the pancreas and blocked.A comprehensive exploration of the spleen injury was conducted,revealing a tear at the lower pole of the spleen,with a length of 5.0 cm and about 2.0 cm to the upper pole at the splenic hilum.The upper and lower branches of the splenic artery and the lower branch of the splenic vein were dissected and ligated,while the upper branch of the splenic vein was preserved.The ischemic line of the spleen supplied by the short gastric vessels was observed.Under the guidance of the ischemic line,most of the middle and lower poles of the spleen were dissected and ligated,and the hemostasis effect on the wound surface was satisfactory.The modified Warshaw partial splenectomy was completed.
