Laparoscopic mesohepatectomy with ICG negative staining

Abstract:Anatomic mesohepatectomy is an important surgical approach for malignant tumors located in the central area of the liver.With the rapid development of laparoscopic technology in hepatobiliary surgery,anatomic mesohepatectomy can now be performed with laparoscopy.The surgery involves almost all important structures of the liver.Therefore,it can be regarded as a highly difficult milestone surgical approach in the field of laparoscopic hepatectomy.Indocyanine green(ICG)fluorescence imaging laparoscopy is used in hepatectomy.With the natural anatomical landmark,the transection plane of the left liver can adopt the liver parenchyma priority approach.The right side of ligamentum teres hepatis is used to split the liver parenchyma,expose the sagittal part and the root of the middle hepatic vein,which can obtain the left liver transection.When transecting the right liver,the laennec membrane level can be reached by entering the bottom of the gallbladder to dissect out the right anterior hepatic pedicle and block it,then,ICG negative staining can be used to guide the right liver transection.When combined with fluorescence technology and intraoperative ultrasound,laparoscopic anatomic hepatectomy can be more precise.
