Laparoscopic Duodenum-Preserving Pancreatic Head Resection

Abstract:This report presents a case of a 20-year-old female patient admitted to the hospital due to"one-week history of an abdominal mass discovered during a physical examination."The patient was admitted to hospital and diagnosed as solid pseudopiloma pseudopapillary tumor of the pancreas with clear indications for surgery,and performed laparoscopic duodenum-preserving pancreatic head resection(LDPPHR)under general anesthesia.During the operation,the pancreatic head area was dissected by ultrasound knife,focusing on protecting the blood supply of the gastroduodenal artery,anterior/posterior pancreaticoduodenal arterial arcades,and the common bile duct.The pancreatic head tissue was completely removed and the anatomical continuity of the duodenum and biliary tract was preserved.Key surgical steps included exposure of the pancreatic head inferior margin,dissection of the superior mesenteric vein(SMV),transection of the pancreatic neck,bile duct-guided pancreatic head resection,and the repairing of duodenal serosal.No complex digestive reconstruction was required postoperatively.LDPPHR minimized anatomical disruption,reduced surgical trauma,and effectively improved the patient's quality of life by preserving duodenal perfusion and biliary integrity.Successful outcomes relied on the meticulous protection of the posterior pancreaticoduodenal arterial arcades,particularly on the superior and inferior posterior pancreaticoduodenal arteries.Combined with literature reviews,this study highlights the safety and efficacy of LDPPHR for benign and borderline pancreatic head tumors.
