Indocyanine green fluorescence guided laparoscopic anatomical S5+6v hepatectomy plus cholecystectomy

Abstract:The laparoscopic anatomical hepatectomy guided by visualization technology is currently the mainstream surgical method for liver tumors,and has the advantage of accurately achieving the resection of the target liver area and improving the prognosis of tumor patients.This case was a primary liver cancer[China Liver Cancer Clinical Staging(CNLC)Ⅱa,Barcelona Clinic Liver Cancer Staging(BCLC)A].Through preoperative computerized tomography(CT)and magnetic resonance imaging(MRI),it was determined that the tumor lesions were located in the S5 and S6 ventral branch(S6v)region of the liver.Through three-dimensional reconstruction,the relationship between the tumor and vessels was determined,and the tumor portal territory analysis was performed to develop a simulated resection plan.During surgery,ultrasound was used to determine the location and nature of the lesions,and Glission branch of S5(G5)and S6 ventral branch(G6v)were precisely disconnected to perform indocyanine green(ICG)fluorescence negative staining in the pre-resected liver segment to determine the pre-resected region.Finally,combined with intraoperative ultrasound,fluorescence and landmark of the hepatic vein,the laparoscopic anatomical hepatectomy of S5+6 v was performed.
