Application and therapeutic effect of stepwise psychological intervention in patients with chronic traumatic amputation of toes(limbs)

Abstract：Objective To explore the application effect of stepwise psychological intervention in patients undergoing chronic invasive toe amputation surgery and its impact on psychological stress response.Methods Sixty patients who underwent chronic invasive toe(limb)amputation surgery from March 2021 to June 2022 in Burn Plastic Surgery Department,Changzhou Second People's Hospital,Nanjing Medical University were selected as the subjects,and were divided into two groups using the envelope method,with 30 cases in each group.The control group received routine nursing care,while the observation group received a combination of stepwise psychological intervention.Both groups completed a 6-week intervention.The self-protection awareness,hope level,psychological stress response,postoperative complications and satisfaction of the two groups were compared.Results After 6 weeks of intervention,the observation group had higher scores in terms of self-awareness,health knowledge,sense of responsibility,self-care skills,and attitude,action and intimacy in the Self Protection Ability Assessment(ESCA)and the Herth Hope Scale(HHI)compared with the control group(P<0.05).After 6 weeks of intervention,the psychological stress test scale(PSTR),Hamilton Depression Scale(HAMD)and Hamilton Anxiety Scale(HAMA)scores of the observation group were lower than those of the control group(P<0.05).There were no statistically significant differences in postoperative wound infection,phantom limb pain,and skin necrosis between the two groups(P>0.05).The satisfaction level of the observation group after 4 weeks of intervention was higher than that of the control group(P<0.05).Conclusion Stepwise psychological intervention used in patients undergoing chronic toe amputation surgery can improve their self-protection awareness and hope level,reduce psychological stress reactions,and do not increase the incidence of postoperative complications.It can achieve high satisfaction and is worth promoting and applying.
