Retrospective analysis on perioperative body temperature maintenance in patients undergoing open reduction and internal fixation of ankle fractures

Abstract:Objective To investigate the effect of comprehensive insulation measures on body temperature maintenance during open reduction and internal fixation of ankle fractures.Methods A total of forty patients with ankle fractures who received open reduction and internal fixation were retrospective analyzed.Group A was comprehensive insulation group,in which patients were covered with 3 M inflatable thermal insulation blankets,intravenous fluids were warmed to 37℃before intravenous injection,and rinse fluid was also warmed to 37℃before use.The operation room temperature was also set at 24℃before patients entered.Group B was control group,in which patients were only covered with thin quilt and none of other active warming method was used.The core temperature of two groups were recorded after patients entering the surgery room(T0),before anesthesia(T1),and every 15 minutes since the surgery began(T 2:15 min,T 3:30 min,T 4:45 min,T 5:60 min,and T 6:end of operation).Operation time,anesthesia time,intraoperative blood loss,transfusion volume,flushing fluid amount,drainage 24 hours after surgery,recovery time,and extubation time were recorded for both groups.The incidences of shivering and hypothermia during anesthesia recovery were observed.Patient thermal comfort degree and surgeon comfort level were compared between two groups.Results The core temperatures at T 0,T 1 and T 2 between two groups had no differences(P>0.05).But the core temperatures at T3,T4,T5 and T6 between two groups had statistically significant differences(P<0.05).The drainage 24 hours after surgery,recovery time,extubation time and incidence of hypothermia of group A were different from those of group B(P<0.05).The patients in group A had higher thermal comfort degree than patients in group B,but the surgeons felt the operation room temperature in group A was higher than that in group B(P<0.05).Conclusion Comprehensive insulation measures can keep the core temperature stable during surgery for patients with ankle fractures.It can also reduce recovery time,incidence of shivering and drainage after surgery.Comprehensive insulation measures can ensure operation safety.
