Analysis of the prognosis and influencing factors of 29 cases of transvaginal cervical cerclage

Abstract:Objective To analyze the prognosis and the factors affecting the therapeutic effect of transvaginal cervical cerclage.Methods The clinical data of 29 patients with single pregnancy who were hospitalized in the Obstetrics Department of our Hospital from January 2018 to December 2022 were retrospectively analyzed,and their pregnancy outcomes were tracked.According to the pregnancy outcome,two groups were divided:the late pregnancy group(≥28 weeks of pregnancy during delivery)and the abortion group(<28 weeks of pregnancy during delivery).The general data and observation indexes of the two groups of pregnant women were analyzed statistically.Results There were statistically significant differences between the two groups in preoperative cervical canal length,cervical canal dilation,surgical indications,prolongation of gestational age after cervical cervicectomy and delivery gestational age(P<0.05).Binary logistic regression analysis showed that preoperative cervical canal length(P<0.05,OR=4.148)and cervical canal dilation(P<0.05,OR=8.128)were prognostic factors.There were no significant differences in risk factors related to maternal age,BMI,gestational times,abortion times,the gestational week of cervical cerclage,reproductive tract infection,uterine operation history or cervical operation history(P>0.05).Conclusion The success rate of transvaginal cervical cerclage based on ultrasound as the surgical indication may be higher than that of the other two surgical indications.The length and dilation of the cervical canal before operation are the prognostic factors.In clinical practice,it is of great significance to strengthen ultrasonography for high-risk patients and perform transvaginal cervical cerclage in time to prevent late abortion and improve pregnancy outcome.
