Endovascular treatment of high-flow direct carotid-cavernous sinus fistula:a case report and literature review

Abstract:Carotid-cavernous sinus fistula (CCF) is a clinical syndrome caused by abnormal arteriovenous communication between the carotid artery and its branches and the cavernous sinus. According to the cause, CCF is divided into traumatic and spontaneous types. Traumatic carotid-cavernous sinus fistula (TCCF) refers to a clinical syndrome caused by trauma leading to rupture of the main trunk or branches of the carotid artery in the cavernous sinus segment, resulting in an abnormal arteriovenous communication between the carotid artery and the cavernous sinus [1]. The causes of trauma mainly include head and face injuries (especially basilar skull fractures), iatrogenic injuries (such as endovascular treatment, cavernous sinus surgery), and injuries caused by transsphenoidal surgery that damage the internal carotid artery in the cavernous sinus segment [2]. This article reports a case of high-flow direct CCF caused by trauma. The patient had a large fistula, and after treatment with a Willis covered stent, the therapeutic effect was satisfactory. The case is reported here.
