Explore the optimal antihypertensive scheme in the acute period of ischemic stroke
Abstract:Objective To explore the optimal antihypertensive scheme in the acute period of ische‐mic stroke .Methods A total of 873 patients with first acute ischemic stroke within 24 hours of onset were enrolled , and were divided into observation group (n = 132) , intravenous antihypertensive group (n = 88) and oral antihypertensive group (n = 653) according to whether taken antihypertensive therapy and the drug route .On the basis of conventional treatment of ischemic stroke ,the intravenous antihypertensive group received intravenous antihypertensive drugs ,the oral group was given oral antihy‐pertensive drugs ,while the observation group was not .National Institutes of Health Stroke USA Scale (NIHSS) was used to evaluate the degree of neurologic deficit and the modified Rankin Scale (mRS) was used to assess the prognosis function .Results Logistic regression analysis showed that the patients trea‐ted with calcium channel blockers (CCB ) combined with angiotensin - converting enzyme inhibitors (ACEI) or angiotensin receptor blockers (ARB) combined with β receptor antagonist (β -B) had the highest probability in neurologic good recovery (OR =1 .894 4 ,95% CI =1 .420 8-2 .368 0 ,P= 0 .038 8) , the lowest risk in neurological deterioration and dysfunction (OR =0 .438 6 and 0 .587 8 ,refer to CCB) . These results still existed after multiparameter adjusted (refer to CCB) .Conclusions Reasonable antihy‐pertensive treatment in acute phase of ischemic stroke is beneficial to the prognosis of the patients ,and the combined antihypertensive drugs have more advantages .The prognosis of CCB+ACEI/ARB+β-B is the best .
