Effect of Concurrent Versus Sequential Chemoradiotherapy on Prognosis of Esophageal Cancer:A Meta-analysis

Abstract:Objective To systematically evaluate the effect of concurrent chemoradiotherapy(CCRT)versus sequential chemoradiotherapy(SCRT)on the prognosis of esophageal cancer.Methods Cochrane,PubMed,Embase,Web of Science,China Biomedical Literature Database(CBM),China National Knowledge Infrastructure(CNKI),VIP and Wan fang Database were searched,and the articles were screened according to the inclusion and exclusion criteria.NOS scale was used to evaluate the methodological quality of the included cohort studies,and Review Manager 5.4 software was used for meta-analysis.Results A total of 32 cohort studies with 2 962 patients were included.The objective response rate[OR= 2.82,95%CI(2.16~3.68),P<0.000 01]and 1-,2-,and 3-year overall survival rate(P<0.000 01)in the CCRT group were better than those in the SCRT group,and there was no significant difference in the dis-tant metastasis rate[OR=0.82,95%CI(0.55~1.23),P=0.33].The incidence of leucopenia[OR=2.90,95%CI(1.79~4.70),P<0.000 1]and radiation esophagitis[OR = 1.90,95%CI(1.46~2.47),P<0.000 01]in the CCRT group was higher than that in the SCRT group.Conclusion In the CCRT group,the objective response rate and 1-,2-,3-year overall survival rate are better than those in the SCRT group,but the incidence of adverse reactions such as leukopenia and radiation esophagitis is higher.
