Application effect of closed-loop target-controlled infusion of propofol in anesthesia for percutaneous balloon compression of the trigeminal ganglion

Abstract:Objective To observe the application effect of closed-loop target-controlled infusion of propofol in an-esthesia for percutaneous balloon compression of the trigeminal ganglion.Methods Sixty-three patients with trigeminal neuralgia(TN)undergoing percutaneous balloon compression were prospectively selected and were randomly divided into the observation group(n=32)and control group(n=31).Both groups received target-controlled infusion(TCI)of propofol during balloon compression,with a target bispectral index of 50-60.In the observation group,the target concentration of propofol was automatically adjusted by a closed-loop system,while in the control group,it was manually adjusted by anes-thesiologists according to vital signs.Blood pressure and pulse were recorded at admission to the operating room(T0),loss of consciousness(T1),start of surgery(T2),balloon compression(T3),and end of surgery(T4).Visual analogue scale(VAS)scores were assessed before surgery and 1 day after surgery.Intraoperative propofol dosage,postoperative awakening time,balloon compression time,and balloon inflation volume were recorded and compared between the two groups.Results Compared with T0,blood pressure increased significantly and pulse rate decreased significantly at T1 and T2 in the observation group(both P<0.05),while similar changes were observed at T1,T2,and T3 in the control group(all P<0.05).At T1,T2,and T3,blood pressure was significantly lower,and pulse rate was significantly higher in the observation group than in the control group(all P<0.05).One day after surgery,VAS scores were significantly lower than those before surgery in both groups,and the score in the observation group was significantly lower than that in the con-trol group(all P<0.05).The intraoperative propofol dosage was significantly lower,and postoperative awakening time was significantly shorter in the observation group than in the control group(both P<0.05).In addition,balloon compression time and balloon inflation volume were significantly lower in the observation group than in the control group(both P<0.05).Conclusion Closed-loop target-controlled infusion of propofol can effectively maintain intraoperative hemody-namic stability in patients with TN undergoing percutaneous balloon compression,reduce anesthetic consumption,shorten postoperative awakening time,alleviate early postoperative pain,and improve surgical efficiency by optimizing balloon compression parameters.
