Research progress on mechanism of HIF-1α in pathogenesis of acute respiratory distress syndrome

Abstract:Acute respiratory distress syndrome(ARDS)is a critical clinical syndrome characterized by diffuse lung injury and severe pulmonary dysfunction.It has a complex pathophysiology,a high mortality rate,and lacks specific thera-peutic interventions.Hypoxia-inducible factor-1α(HIF-1α)is a key transcription factor mediating cellular responses to hypoxia and is considered to play a pivotal role in the initiation,progression,and prognosis of ARDS.Under combined hy-poxic and inflammatory stimuli,HIF-1α is markedly up-regulated,promoting the release of proinflammatory cytokines and aberrant activation of immune cells,thereby exacerbating pulmonary inflammation.Concurrently,HIF-1α aggravates lung tissue injury by regulating macrophage polarization,neutrophil function,and T-lymphocyte differentiation.HIF-1α also fa-cilitates metabolic reprogramming by enhancing glycolysis and lactate production,inducing oxidative stress and amplifying inflammatory responses,which in turn promotes apoptosis of alveolar epithelial and endothelial cells and disrupts the alveo-lar-capillary barrier.In addition,HIF-1α accelerates the progression of pulmonary fibrosis by promoting fibroblast activa-tion,epithelial-mesenchymal transition,and M2 macrophage polarization.A comprehensive understanding of the mecha-nisms by which HIF-1α contributes to ARDS pathogenesis will provide a theoretical basis for elucidating the pathological foundations of ARDS and for the development of novel therapeutic strategies.
