Influence of renal function status on efficacy of intravitreal injection of ranibizumab in patients with diabetic macular oedema

Abstract:Objective To investigate the impact of renal function status on the efficacy of intravitreal injection of ra-nibizumab in patients with diabetic macular oedema(DME),thereby providing a basis for individualised clinical treat-ment.Methods Seventy-six patients with DME(95 eyes)were selected and divided into three groups based on estimat-ed glomerular filtration rate(eGFR):group A(normal renal function,26 patients,34 eyes),group B(mild renal impair-ment,26 patients,31 eyes),and group C(renal insufficiency,24 patients,27 eyes).All affected eyes received intravitre-al injection of 0.5 mg/0.05 mL ranibizumab using a 3+PRN injection regimen.Best-corrected visual acuity(BCVA)and central macular thickness(CMT)were measured preoperatively and at 1,2,and 3 months postoperatively.The number of eyes with DME recurrence was recorded,and recurrence rates were calculated.Results Preoperative BCVA and CMT showed no statistically significant differences between groups(all P>0.05).At 1,2,and 3 months postoperatively,BCVA and CMT decreased in all groups in comparison with the preoperative levels,with group C>group B>group A at each time point(all P<0.05).The DME recurrence rates in the groups A,B,and C were 21.2%,46.9%,and 76.7%,respective-ly;the recurrence rates of DME followed the order:group C>group B>group A(all P<0.05).Conclusions Renal func-tion status significantly influences the therapeutic efficacy of intravitreal injection of ranibizumab in DME patients.Higher eGFR correlates with greater visual acuity improvement,more complete resolution of macular oedema,and lower recur-rence risk.
