Short-term efficacy of self-formulated Guben Qingre Paishi decoction on residual stones after flexible ureteroscopic lithotripsy in patients with upper urinary tract stones of kidney deficiency and damp-heat type

Abstract:Objective To observe the short-term efficacy and safety of the self-designed Guben Qingre Paishi decoc-tion in treating residual stones(referred to as"residual stones")in the upper urinary tract after ureteroscopic lithotripsy for patients with upper urinary tract stones of kidney deficiency and damp-heat type.Methods A total of 140 patients with upper urinary tract stones who had residual stones after ureteroscopic lithotripsy and were diagnosed as the syndrome of kid-ney deficiency and damp-heat type by traditional Chinese medicine were selected.They were randomly divided into the treatment group and the control group,with 70 cases in each group.Patients in the control group received conventional Western medicine treatment starting from 2 days after the operation,while patients in the treatment group were given oral administration of the self-designed Guben Qingre Paishi decoction on this basis,and the treatment was continued for 4 weeks.The residual stone clearance rate,clinical efficacy,stone sediment grade in the ureteral stent,time of residual stone expulsion,Chinese medicine syndrome scores before treatment and at 4 weeks of treatment,rehabilitation conditions before treatment and at 1,2,and 4 weeks of treatment,and the number of red blood cells(RBC)and white blood cells(WBC)in the urine before treatment and at 1,2,and 4 weeks of treatment,as well as safety-related indicators[serum RBC,hemoglobin(Hb),alanine aminotransferase(ALT),aspartate aminotransferase(AST),creatinine(Scr),urea ni-trogen(BUN),and the occurrence of renal colic,hematuria,fever and other complications during treatment for 4 weeks]were compared between the two groups.Results The residual stone clearance rate and total effective rate in the treat-ment group were higher than those in the control group;the stone deposition situation in the ureteral stent of the treatment group was better than that of the control group,and the time for residual stone expulsion was shorter in the treatment group than in the control group(all P<0.05).Compared with the condition before treatment in the same group,the TCM syn-drome scores of both groups decreased after 4 weeks of treatment,while the rehabilitation scores increased after 1 week of treatment;moreover,the changes in the treatment group were more significant(all P<0.05).Compared with the pre-treat-ment of the same group,the number of WBC and RBC in urine of the treatment group significantly decreased after 2 and 4 weeks of treatment(all P<0.05).In the control group,the number of WBC and RBC in urine decreased after 2 weeks of treatment,while the number of WBC in urine decreased after 4 weeks of treatment,but the number of RBC increased;compared with the control group at the same time point,the number of WBC and RBC in urine of the treatment group im-proved after 2 and 4 weeks of treatment(all P<0.05).There were no statistically significant differences in RBC,Hb,ALT,AST,BUN or Scr between the two groups after 4 weeks of treatment(all P>0.05).The incidence of complications in the treatment group was lower than that in the control group(P<0.05).Conclusion For patients with residual stones in the upper urinary tract after ureteroscopic lithotripsy for kidney and damp-heat syndrome,taking the self-designed Guben Qingre Paishi decoction can promote the expulsion of stones,increase the clearance rate of stones,reduce the depo-sition of accessory tube stones,and is conducive to postoperative recovery with higher safety.
