Efficacy and safety of high-dose versus low-dose radiotherapy in concurrent chemoradiotherapy for esophageal squamous cell carcinoma

Abstract:Objective To evaluate the efficacy and safety between high-dose and low-dose radiotherapy in concur-rent chemoradiotherapy for esophageal squamous cell carcinoma.Methods From 2017 to 2020,63 patients with esopha-geal squamous cell carcinoma undergoing concurrent chemoradiotherapy were divided into the high-dose group of 42 cases(60-70 Gy)and the low-dose group of 21 cases(50-50.4 Gy).We recorded and compared the overall survival(OS),1-year overall survival rate,3-year overall survival rate,progression-free survival(PFS),1-year progression-free survival rate,3-year progression-free survival rate,as well as the occurrence of complications such as acute radiation esophagitis,acute radiation pneumonia,and granulocytopenia.Results The OS of the high-dose group was 30.00 months(95%CI:12.34-47.67),the 1-year and 3-year OS rates were 76.2%and 45.1%,the PFS was 19.00 months(95%CI:13.56-24.44),the 1-year and 3-year PFS rates were 61.9%and 33.3%;the OS of the low-dose group was 16.00 months(95%CI:14.29-17.71),the 1-year and 3-year OS rates were 75.4%and 30.2%,the PFS of the low-dose group was 12.00 months(95%CI:9.78-14.22),the 1-year and 3-year PFS rates were 42.9%and 16.7%;there was no significant differ-ence between the two groups(all P>0.05).During the follow-up period,22 patients(52.4%)in the high-dose group ex-perienced disease progression,including 11 cases of pure local recurrence,4 cases of pure distant metastasis,and 7 cases of both local recurrence and distant metastasis;in the low-dose group,12 patients(57.1%)experienced disease progres-sion,including 8 cases of pure local recurrence,3 cases of pure distant metastasis,and 1 case of both local recurrence and distant metastasis;there was no significant difference between the two groups(all P>0.05).The incidence rates of acute radiation esophagitis,acute radiation pneumonia,and granulocytopenia in the high-dose group were 40.4%,23.8%,and 52.4%,respectively,while in the low-dose group they were 42.9%,14.3%,and 42.9%,respectively;there was no significant difference between the two groups(all P>0.05).Conclusion Compared with low-dose(50-50.4 Gy)radiotherapy,high-dose(60-70 Gy)radiotherapy had no favorable survival outcomes for patients with esopha-geal squamous cell carcinoma.
