Clinical efficacy of dapagliflozin in the treatment of non-diabetic elderly patients with chronic obstructive pulmonary disease

Abstract:Objective To observe the clinical efficacy of dapagliflozin in the treatment of non-diabetic elderly pa-tients with chronic obstructive pulmonary disease(COPD).Methods A total of 110 non-diabetic elderly patients with COPD were randomly allocated(random-number table)into the observation group and control group(with 55 patients in each).Patients in the control group received conventional therapy,including inhaled glucocorticoids,bronchodilators and expectorant/antitussive agents,while patients in the observation group received the same regimen plus oral dapa-gliflozin(10 mg,once daily,15 min before breakfast)for 28 days.Before and after treatment,pulmonary function[forced vital capacity(FVC),forced expiratory volume in 1 second(FEV1),and FEV1/FVC],cardiac function[left-ven-tricular ejection fraction(LVEF),left-ventricular end-diastolic diameter(LVEDD),and left-ventricular end-systolic diame-ter(LVESD)],inflammatory indices[fractional exhaled nitric oxide(FeNO),C-reactive protein(CRP),and procalcitonin(PCT)],humoral immunity[immunoglobulin A(IgA),IgG,IgM],and adverse events were compared between groups.Results After treatment,both groups showed significant increases in FVC,FEV1,FEV1/FVC,LVEF and serum IgA/IgG/IgM,and significant decreases in LVEDD,LVESD,FeNO,CRP and PCT as compared with their respective base-lines(all P<0.05).Compared with the control group,the observation group demonstrated greater improvements in FVC,FEV1,FEV1/FVC,LVEF and serum IgG/IgM,and greater reductions in LVEDD,LVESD,FeNO,CRP and PCT(all P<0.05).Adverse event rates did not differ significantly between the observation group[8/55,14.55%;nausea/vomiting 2(3.64%),rash 3(5.45%),muscle tremor 2(3.64%),and headache 1(1.82%)]and the control group[6/55,10.91%;nausea/vomiting 2(3.64%),rash 2(3.64%),muscle tremor 1(1.82%),and headache 1(1.82%)](all P>0.05).Conclusion Dapagliflozin effectively controls airway inflammation,reduces inflammatory-mediator levels,and improves pulmonary,cardiac and humoral immune functions in non-diabetic elderly COPD patients without increasing the risk of notable adverse reactions,indicating good safety.
