Predictive efficacy of PhA measurement for short-term efficacy of arthroscopic meniscoplasty in elderly patients with meniscus injury

Abstract:Objective To evaluate the predictive value of the phase angle(PhA)measured by bioelectrical impedance analysis for the short-term efficacy of arthroscopic meniscoplasty in elderly patients with meniscus injury.Methods Sixty elderly patients with meniscus injury who underwent arthroscopic meniscoplasty were divided into the excellent-good group(44 cases)and fair-poor group(16 cases)based on the short-term therapeutic effect.General data of the patients,includ-ing age,gender,body mass index(BMI),disease course,affected side,and history of trauma,were collected.The dis-ease data of the patients,such as posterior horn injury of the medial meniscus,degree of knee cartilage injury,synovial cleaning,and occurrence of complications,were also collected.The results of bioelectrical impedance analysis of body composition were obtained,including intracellular water,extracellular water,total body water,affected limb segment wa-ter,body cell mass,fat-free mass,fat-free BMI,fat tissue mass,fat tissue index,muscle mass,muscle mass index,mus-cle and PhA of the affected limb segment.Univariate analysis and binary Logistic regression analysis were used to analyze the influencing factors of surgical efficacy.The predictive efficacy of PhA for surgical effect was analyzed by receiver operat-ing characteristic(ROC)curve.Results BMI,the proportion of disease duration>1 year,the proportion of patients with medial posterior horn meniscus injury and the proportion of patients with severe cartilage injury were higher in the fair-poor group than in the excellent-good group,while PhA,intracellular water,and muscle mass index were lower in the fair-poor group than in the excellent-good group(all P<0.05).Logistic regression analysis showed that lower PhA(OR=9.565,95%CI:2.005-25.637,P=0.005)and disease duration>1 year(OR=0.055,95%CI:0.004-0.778,P=0.032)were independent risk factors for poor short-term outcomes of arthroscopic surgery in elderly patients with meniscus injury.The area under the ROC curve of PhA in predicting poor surgical outcomes was 0.887(95%CI:0.796-0.978,P<0.01),with the sensitivity of 0.932,specificity of 0.750,and cut-off value of 4.4°.Conclusion PhA has a good predictive val-ue for the short-term efficacy of arthroscopic surgery in elderly patients with meniscus injury.
