Lymph node metastasis and risk factors of patients with early intestinal-type gastric adenocarcinoma

Abstract:Objective To observe the lymph node metastasis in patients with early intestinal-type gastric adenocarci-noma,and to analyze its risk factors.Methods Sixty-one patients with early intestinal-type gastric adenocarcinoma who underwent radical resection of gastric cancer were retrospectively selected,and the lymph node metastasis detected during surgery was recorded.The gender,age,histological differentiation,tumor diameter,tumor location,depth of invasion,vascular cancer thrombus and nerve invasion among patients with lymph node metastasis and non-metastasis were com-pared,and multivariate Logistic regression was used to analyze the independent risk factors for lymph node metastasis in patients with early intestinal-type gastric adenocarcinoma.Results Among the 61 patients with early intestinal-type gas-tric adenocarcinoma,15(24.6%)patients had lymph node metastasis and 46(75.4%)patents had no lymph node metas-tasis.There were no statistically significant differences in gender,age,histological differentiation degree,tumor location or presence of vascular cancer emboli between the early intestinal-type gastric adenocarcinoma patients with and without lymph node metastasis(all P>0.05).The proportions of tumor diameter>3 cm,depth of invasion and nerve invasion in the early intestinal-type gastric adenocarcinoma patients with lymph node metastasis were higher than those in the patients without lymph node metastasis(all P<0.05).The results of multivariate Logistic regression analysis showed that tumor di-ameter>3 cm and depth of invasion were independent risk factors for lymph node metastasis in patients with early intesti-nal-type gastric adenocarcinoma(OR=8.383,95%CI:1.382-50.859;OR=16.455,95%CI:1.783-151.876,all P<0.05),while nerve invasion was not an independent risk factor for lymph node metastasis in patients with early intestinal-type gastric adenocarcinoma(OR=5.519,95%CI:0.944-32.280,P>0.05).Conclusion Lymph node metastasis ex-ists in some patients with early intestinal-type gastric adenocarcinoma,and tumor diameter>3 cm and depth of invasion are risk factors for lymph node metastasis in these patients.
