Effects of HPV 16 infection in semen of patients with severe oligoasthenozoospermia on sperm and pregnancy outcome of ICSI-ET

Abstract:Objective To investigate the impact of human papillomavirus 16(HPV16)infection in semen of pa-tients with severe oligoasthenozoospermia on sperm and pregnancy outcome of intracytoplasmic sperm injection-embryo transfer(ICSI-ET).Methods Eighty-two male patients with severe oligoasthenozoospermia were selected and divided into the positive group(only HPV 16 positive)and negative group(HPV negative)according to the semen HPV test re-sults.Both groups were given ICSI-ET for pregnancy.The integration of HPV16 DNA in the nucleus of sperm head was de-tected by fluorescence in situ hybridization,and the sperm DNA fragmentation rate was calculated by sperm chromatin dif-fusion method.The related indicators of ovulation promotion[gonadotropin(Gn)use time,Gn dosage,estradiol on trigger day,the number of mature eggs,and the endometrial thickness on embryo transfer day)of the two groups were counted,and the pregnancy outcome indicators(normal fertilization rate,the number of high-quality embryos on the third day after egg retrieval,implantation rate,clinical pregnancy rate,and abortion rate)of the two groups were counted.Results In the positive group,HPV 16 DNA integration was found in the sperm head nucleus of 12 patients,and the integration rate was 29.3%.HPV DNA integration was not found in the nucleus of sperm head in the negative group.The sperm DNA frag-mentation rates in the positive and negative groups were 26.29%±2.18%and 23.46%±2.07%,respectively,and the sperm DNA fragmentation rate in the positive group was higher than that in the negative group(P<0.05).There was no significant difference in the related indicators of ovulation induction between the two groups(P>0.05).There was no sig-nificant difference in the normal fertilization rate between the two groups(P>0.05).The number of high-quality embryos,implantation rate and clinical pregnancy rate on the third day after oocyte retrieval in the positive group were lower than those in the negative group(all P<0.05),and the abortion rate was higher than that in the negative group(P<0.05).Conclusions Sperm DNA fragmentation rate significantly increases in severe oligozoospermic patients with HPV 16-posi-tive semen,with some patients exhibiting HPV DNA integration.Even after sperm washing,complete removal of HPV DNA is not achieved,leading to a notable decrease in high-quality embryos,implantation rates,and clinical pregnancy rates,along with an increase in miscarriage rates.
