Comparison of argatroban versus citrate anticoagulation in continuous renal replacement therapy for patients with acute severe pancreatitis

Abstract:Objective To compare the efficacy of argatroban versus citrate anticoagulation in continuous renal replace-ment therapy(CRRT)for patients with severe acute pancreatitis(SAP).Methods A retrospective study was conducted on 80 SAP patients,who were divided into the argatroban group of 35 cases and citrate group of 45 cases based on the anticoagu-lant used during CRRT.The study compared the single filter lifespan,coagulation function parameters[activated partial thromboplastin time(APTT),prothrombin time(PT),fibrinogen(FIB),and D-dimer(D-D)],serum electrolyte levels[sodi-um ion(Na+),potassium ion(K+),and calcium ion(Ca2+)],and economic indicators(hospital stay duration,total hospitaliza-tion cost,daily cost)before and within 24 h after CRRT.Results The single filter lifespan in the argatroban group was short-er than that in the citrate group(P<0.05).After treatment,D-D levels increased in both groups compared to baseline(both P<0.05).The citrate group exhibited lower APTT and PT levels but a higher D-D level than the argatroban group(all P<0.05).After treatment,the citrate group showed a reduced K+level compared to the baseline(P<0.05),and the Ca2+lev-el was lower in the citrate group than in the argatroban group(P<0.05).The argatroban group had shorter hospital stay,low-er total hospitalization cost,and lower daily cost than the citrate group(all P<0.05).Conclusion For SAP patients under-going CRRT,argatroban demonstrates superior anticoagulant efficacy and cost-effectiveness in comparison with citrate.
