Application effect of rehabilitation forward concept in rehabilitation of patients with severe knee osteoarthritis after unilateral total knee arthroplasty

Abstract:Objective To observe the application effect of the concept of rehabilitation forward movement in the postoperative rehabilitation of patients with severe knee osteoarthritis(KOA)after unilateral total knee arthroplasty(TKA).Methods Ninety-four patients with severe KOA were selected and randomly divided into the control group and the observation group according to the double-blind random number table method,with 47 cases in each group.Patients in both groups underwent unilateral TKA treatment.Patients in the control group were given routine care,while patients in the observation group were given the concept of rehabilitation forward movement care.The knee joint pain was evaluated by Visual Analogue Scale(VAS)score before surgery and at 24 h,1,and 2 weeks after surgery.The knee joint function was evaluated by the Hospital for Special Surgery(HSS)score.The active range of motion(AROM)of knee joint flexion/ex-tension was measured by electronic goniometer at 1 and 2 weeks after surgery.The quality of life was evaluated by the Ar-thritis Impact Measurement Scales 2-Short Form(AIMS2-SF)before surgery and at 2 weeks after surgery.The hospitaliza-tion time and postoperative complications of the two groups were statistically analyzed.Results At 24 h,1,and 2 weeks after surgery,the VAS scores of the observation group were all lower than those before surgery(all P<0.05),and were low-er than those of the control group at the same time(all P<0.05).At 1 and 2 weeks after surgery,the VAS scores of the control group were all lower than those before surgery(all P<0.05).At 1 and 2 weeks after surgery,the HSS scores of both groups were higher than those before surgery(all P<0.05),and the HSS scores of the observation group were higher than those of the control group at the same time(all P<0.05).At 2 weeks after surgery,the HSS scores of both groups were higher than those at 1 week after surgery(all P<0.05).At 2 weeks after surgery,the AROM of knee joint flexion/ex-tension of both groups was higher than that at 1 week after surgery(all P<0.05),and the AROM of the observation group was higher than that of the control group(all P<0.05).At 2 weeks after surgery,the physical,symptom,and impact scores of the observation group were all higher than those before treatment(all P<0.05),and were higher than those of the control group at the same time(all P<0.05).At 2 weeks after surgery,the physical,symptom,and impact scores of the control group were all higher than those before treatment(all P<0.05).The hospitalization time of the observation group was shorter than that of the control group(P<0.05),and the incidence of postoperative complications was lower than that of the control group(P<0.05).Conclusion The concept of rehabilitation forward movement can reduce the pain of pa-tients with severe KOA after TKA,improve knee joint function and range of motion,improve quality of life,shorten hospi-talization time,and reduce postoperative complications.
