Clinical efficacy of Shenfu injection in extracorporeal cardiopulmonary resuscitation for cardiac arrest patients

Abstract:Objective To observe the clinical efficacy of Shenfu injection(SFI)in patients with cardiac arrest(CA)undergoing extracorporeal cardiopulmonary resuscitation(ECPR).Methods Sixty CA patients were divided into the obser-vation group and control group(n=30 each)through propensity score matching analysis.Patients in both groups received con-ventional cardiopulmonary resuscitation(CPR)upon admission.ECPR was initiated when spontaneous circulation remained unestablished after 30 minutes of CPR.The observation group received intravenous infusion of 100 mL SFI+250 mL 0.5%glu-cose solution twice daily for 7 consecutive days after successful ECPR establishment.The degree of disturbance of conscious-ness was assessed using Glasgow Coma Scale(GCS)scores at admission and discharge.Survival rates on treatment day 7,15,and 30 were recorded.Clinical timelines including no-flow duration,CA-to-extracorporeal membrane oxygenation(ECMO)establishment interval,ECMO duration,hospitalization duration,and intensive care unit(ICU)stay duration were compared.Adverse events on day 7 were documented.Results At discharge,patients in both groups showed increased GCS scores compared with those at admission(all P<0.05),with lower scores in the observation group(all P<0.05).No sig-nificant differences were found in survival rates between groups on day 7,15,and 30(all P>0.05).The observation group demonstrated shorter hospitalization duration and ICU stay(both P<0.05),while no-flow duration,CA-to-ECMO interval,and ECMO duration showed no intergroup differences(all P>0.05).Adverse event rates did not differ significantly(P>0.05).Conclusion Shenfu injection may improve neurological outcomes and reduce hospitalization/ICU stay duration in ECPR-treated CA patients without increasing adverse events,but it shows no significant impact on short-term survival rates.
