Comparison of prognosis of elderly hip fracture patients at different injury-to-surgery times

Abstract:Objective To compare the prognosis of elderly patients with hip fractures at different injury-to-surgery times.Methods A retrospective collection was conducted on 728 elderly patients with hip fractures.According to the time from injury to surgery,they were divided into the observation group(injury-to-surgery time≤48 h)of 291 cases and the control group(injury-to-surgery time>48 h)of 437 cases.Patients in both groups underwent proximal femoral intra-medullary nail fixation or joint replacement surgery,and the surgical time,intraoperative blood loss,and hospital stay were recorded for both groups.We observed and recorded the occurrence of postoperative complications such as anemia,delirium,pneumonia,pressure ulcers,hypoalbuminemia,and venous thrombosis in two groups.Patients were followed up by telephone to observe the incidence of incidence of recurrent fractures and mortality in the two groups one year after surgery.Kaplan-Meier survival curve was utilized to analyze the survival status of the two groups.Results There were no statistically significant difference in the surgical time or intraoperative bleeding between the two groups(both P>0.05).Compared with the control group,the observation group had a shorter hospital stay(P<0.05).Compared with the control group,the incidence rates of postoperative anemia,pneumonia,and pressure ulcers in the observation group were all low-er(all P<0.05).There was no statistically significant difference in the incidence rates of postoperative delirium,hypoal-buminemia,or venous thrombosis between the two groups(all P>0.05).There was no statistically significant difference in the rate of recurrent fractures between the two groups one year after surgery.Compared with the control group,the observa-tion group had a lower mortality rate within one year after surgery(P<0.05).The difference in survival rate between the two groups of patients is statistically significant(P<0.05).Conclusion Elderly hip fracture patients with injury-to-sur-gery time≤48 h have a low risk of death one year after surgery,and shortening the injury-to-surgery time can improve the prognosis of elderly hip fracture patients.
