Application effect of permissive hypercapnia in one-lung ventilation of infants

Abstract:Objective To investigate the effect of permissive hypercapnia (PHC) on inflammation of infants undergoing one-lung ventilation (OLV).Methods Sixty-four infants undergoing selective pulmonary cystadenoma resection were randomly divided into two groups:group C (normocapnia,n =32) and group H (hypercapnia,n =32).Arterial blood gas samples were collected at four different time points:10 minute after intubation (T1),30 minute,60 minute after artificial pneumothorax (T2,T3),and 30 minute after being sent to post anesthesia care unit (T4),and the oxygenation index (OI) was calculated by corresponding equation.Bronchoalveolar lavage fluid (BALF) was collected before and after surgery for the measurement of IL-6 and IL-8 with ELISA.Results Fifty-nine infants were finally included in this study.Compared with T1,PaO2 and OI decreased while Ppeak increased in both groups at T2 and T3;Vt,MVV,PH and Lac decreased while PaCO2 increased in the group H at T2 and T3 (all P < 0.05).Ppeak,Vt,MVV,PH,and Lac decreased while PaCO2 increased in the group H at T2 and T3 as compared with that in the group C (all P < 0.05).There was a significant increase in the IL-6 and IL-8 in both groups after surgery,and this increase was much higher in the group C as compared with that of group H after surgery(all P < 0.05).Conclusion PHC not only has advantages in improving tissue oxygenation,but also has the potential of lung protection for infants undergoing OLV by regulating the inflammation in lung.
