Influencing factors of anemia treatment in maintenance hemodialysis patients

Abstract：Objective To analyze the influencing factors of anemia treatment in maintenance hemodialysis (MHD) patients.Methods Totally 337 cases of patients with end-stage renal disease were divided into the substandard group of 179 cases (Hb ＜ 110 g/L) and standard group of 158 cases (Hb ≥ 110 g/L) according to anemia guidelines.Single factor analysis was used to compare the clinical data of two groups,and the Multi-factor Logistic regression analysis was used to compare the influencing factors for anemia treatment.Results In the standard group,patients with diabetes accounted for 18.4％,with hemoperfusion (HP) frequency more than 2 h/month accounted for 50.6％,patients who took angiotensin converting enzyme inhibitor (ACEI)/angiotensin Ⅱ receptor antagonist (ARB) accounted for 35.4％,the high sensitive C reactive protein (hs-CRP) was (2.21 ± 3.12) mg/L,the serum phosphorus before hemodialysis was (1.93 ±0.65) mmol/L,and the serum albumin before hemodialysis was (39.76 ± 3.11) g/L,patients with transferrin saturation (TSAT) ＞0.2 accounted for 91.8％,and the single chamber urea clearance index (spKt/V) was i.38 ±0.27;in the substandard group,the above indexes were separately 27.9％,35.2％,53.1％,(3.92 ± 4.42) mg/L,(1.77 ± 0.58) mmol/L,(38.38 ±3.38) g/L,82.7％,and 1.30 ±0.29;the differences were statistically significant between these two groups (all P ＜0.05).The SpKt/V,frequency of HP,taking ACEI/ARB,TSAT,hs-CRP,and serum albumin before HD were related to anemia treatment,during which,spKt/V,frequency of HP ＞ 2 h/month,TSAT ＞ 0.2,serum albumin before HD were the protective factors for anemia treatment,and taking ACEI/ARB and hs-CRP were risk factors for anemia treatment.Conclusions Standard-reaching rate of anemia treatment in MHD patients is low.Further more,inadequate dialysis,inadequate blood perfusion,taking ACEI/ARB,iron deficiency,inflammation,and malnutrition may be the influencing factors for anemia treatment.
