Ovarian reserve function and ovarian response in controlled ovarian hyperstimulation of infertile patients with genital tuberculosis and tubal obstruction

Abstract:Objective To observe the ovarian reserve function and the ovarian response in infertile patients with geni-tal tuberculosis and tubal obstruction who received in vitro fertilization-embryo transfer ( IVF-ET) in controlled ovarian hy-perstimulation.Methods The clinical data of 360 infertile patients undergoing IVF for tubal obstruction were analyzed retrospectively.During which, 72 cases were diagnosed with pelvic tubal tuberculosis through surgery ( tuberculosis group);according to the time from the moment of diagnosing with pelvic tubal tuberculosis to receiving IVF treatment, they were then divided into the sub group of 22 cases whose time was ≤5 years and sub group of 50 cases whose time was >5 years.Meanwhile, 288 cases of patients with non-pelvic tubal tuberculosis were taken as the controls ( control group) .O-varian reserve function were evaluated before IVF treatment:the serum FSH, FSH/LH, E2 and antral follicle count ( AFC) and ovarian volume ( OV) were measured on the menstrual cycle 2-3 days;the ovarian response were evaluated during con-trolled ovarian hyperstimulation:the total amount of gonadotropin ( Gn) , Gn time, number of retrieved oocytes and the in-cidence of poor ovarian response were recorded.Results The duration of infertility and Gn were longer, the basal FSH and E2 were higher and AFC, OV and the number of retrieved oocytes were lower in the tuberculosis group as compared with those in the control group (all P<0.05).In the group of tuberculosis, the duration of infertility, the basal FSH were higher, and AFC, OV, the number of retrieved oocytes and E2 on the day of HCG were lower in the sub group of >5 years than those in the sub group of≤5 years (all P<0.05).The incidences of poor ovarian response in the tuberculosis group and the control group were 18.06%and 5.21%, respectively (all P<0.05);and there were no significant difference in the incidence of poor ovarian response between the sub group of >5 years and the sub group of ≤5 years.Conclusion The ovarian reserve declined and poor ovarian response were found in the infertile patients with genital tuberculosis and tub-al obstruction, and the infection time of tuberculosis had adverse effect on the ovarian reserve and ovarian response during ovarian stimulation in IVF.
