Influence of COMTval158 met gene polymorphism on analgesic effect of fentanyl after gynecological operations on patients of Xinjiang Uygur and Han nationality

Abstract:Objective To observe the distribution feature of COMTval 158met gene polymorphism in patients of Xin-jiang Uygur and Han nationality who underwent gynecological operations , and to analyze the relationships of the gene poly-morphism with the antalgic dosage and effect of fentanyl after operation .Methods Eighty patients who underwent selective gynecological surgery with general anesthesia were selected and divided into two groups :the Uygur nationality group ( group U, n=40) and Han nationality group (group H, n=40).The patient controlled intravenous analgesia (PCIA) with fenta-nyl was adopted after operation .PCR-RFLP was adopted to detect the gene polymorphism of COMTval 158 met.The visual analogue scale ( VAS) , Ramasay sedation scores and SpO 2 as well as the total dosage of fentanyl for PCIA within 48 h after operation were observed.Results In the group U, the genotype frequencies of GG , GA and AA were as follows:75.7%, 19%and 5.4%;in the group H, the frequencies were 47.1%, 41.2%and 11.8%;and there were statistically signifi-cant differences between the two groups (all P<0.05).In group U, allele frequency of G accounted for 85.1%and allele frequency of A accounted for 14.9%, while in the group H, they were 67.7% and 32.3%, and statistically significant differences were found between the two groups (all P<0.05).There were no statistically significant differences in SpO 2, Ramasay scores and VAS scores between the two groups (all P>0.05).The total dosage of fentanyl for PCIA within 48 h of group U was higher than that of group H (P<0.05).The total dosage of fentanyl were significantly lower in patients with mutation of COMTval 158 met (GA+AA) than that of the patients with wild GG genotype (all P<0.05).Conclu-sions There are differences between the distribution of COMTval 158met gene polymorphism in patients of Xinjiang Uygur and Han nationality , which may be the main factor that causes the differences in postoperative analgesic effects of fentanyl .
