Research on influencing factors and clinical significance of plasma endothelin-1 levels in children associated with congenital heart defects
Abstract:Femoral venous plasma endothelin-1 concentrations of 69 CHDpatients with left to right shunt were measured by radioimmunoassay.Among them,17 patients complicated with moderate or sever pulmonary hypertension were treated with captopril and their plasma ET-1 levels were also measured after 15 days drug therapy. The results showed patients with left to right shunt had elevated ET-1 concentrations in contrast with normal children(83.46±24.56pg/ml vs 38.53±11.93pg/ml,p<0.001)CHD patients associated with pulmonary hypertension had higher ET-1 levels than those without pulmonary hypertension patients(90.15±22.56pg/ml vs 65.44±6.33pg/ml,p<0.001),and the severer the pulmonary hypertension was,the higher the ET-1 levels were.Oral captopril 1mg/(kg.d) showed the trend to decrease the mean pulmonary pressure(7.78±0.95kPa vs 7.59±0.72kPa)and plasma ET-1 levels(93.27±10.02pg/ml vs 87.32±8.79pg/ml),however,it couldn't reach the statistic levels. These suggested captopril might delay the development of pulmonary hypertension.
