Analysis of Distribution Characteristics of Traditional Chinese Medicine Syndromes in Ovarian Cancer Patients and Their Impacts on Patient's Prognosis

Abstract:Objective:To explore the distribution characteristics of traditional Chinese medicine(TCM)syndromes in ovarian cancer patients and their prognostic value.Methods:A total of 136 ovarian cancer patients treated at our hospital from January 2018 to October 2022 were included in the study.The syndrome information of all patients was collected and analyzed through systematic cluster analysis(CA).Clinical data were compared among patients with different TCM syndromes.Kaplan-Meier survival curves were used to analyze the probability of poor prognosis among patients with different syndromes,and Logistic regression analysis was conducted to examine the relationship between different syndromes and the risk of poor prognosis.Based on postoperative prognosis,patients were categorized into a poor prognosis group(62 cases)and a good prognosis group(74 cases)to explore prognostic factors and construct a nomogram model.Results:The primary symptoms observed in ovarian cancer patients included fatigue,shortness of breath,dizziness,spontaneous sweating,poor appetite,loose stools,pale complexion,dry eyes,pale tongue with a white coating,and a weak,thin pulse.CA revealed five TCM syndromes:qi deficiency,blood deficiency,yin deficiency,qi stagnation and blood stasis,and qi and yin deficiency.Significant differences were observed among the five syndrome types concerning tumor size,FIGO staging,tumor differentiation,and lymph node metastasis(P<0.05).Qi and yin deficiency,FIGO stageⅢ-Ⅳ,poorly differentiated tumors,and lymph node metastasis were identified as independent prognostic factors for poor outcomes in ovarian cancer patients(P<0.05).Patients with qi and yin deficiency syndrome had a significantly higher probability of poor prognosis compared to those with other syndromes(P<0.05).Logistic regression analysis incorporating TCM syndromes indicated that after adjusting for collinear confounding factors,qi and yin deficiency remained independently associated with poor prognosis(P<0.05).The constructed nomogram model demonstrated good discrimination,accuracy,and clinical applicability.Conclusions:Ovarian cancer can be classified into five TCM syndromes:yin deficiency,qi deficiency,qi stagnation and blood stasis,blood deficiency,and qi and yin deficiency.TCM syndromes are closely associated with the prognosis of ovarian cancer patients.The probability of poor prognosis is significantly higher in patients with qi and yin deficiency,which is an independent risk factor for poor outcomes in ovarian cancer patients.
