The value of PD-1 inhibitor immunotherapy combined with Lenvatinib targeted therapy in unresectable hepatocellular carcinoma

Abstract:Objective To investigate the efficacy of PD-1 inhibitor immunotherapy combined with Lenvatinib targeted therapy in the treatment of unresectable hepatocellular carcinoma(uHCC).Methods A total of 50 patients with unresectable hepatocellular carcinoma admitted to our hospital and Jinping County People's Hospital from March 1,2022 to February 28,2025 were selected as the study subjects.A simple randomized controlled trial was conducted,and 25 patients who received PD-1 inhibitor and Lenvatinib immunotherapy combined with targeted therapy were included in the combination group;25 patients who received treatment with Lenvatinib alone were included in the control group.Maintain follow-up,evaluate efficacy using mRECIST criteria,and record the occurrence of adverse reactions and survival status.Results After 12 months of medication,no patients in both groups achieved complete response(CR);The ORR and DCR rates in the combination group were higher than those in the control group(40.00% VS 76.00%,12.00% VS 44.00%,P<0.05).The median duration of PFS and OS in the combined group was 9.5 months and 18.9 months,respectively,while in the control group it was 5.4 months and 11.2 months,respectively.There was a statistically significant difference in the data comparison between the above indicators(P<0.05).In terms of the total incidence of grade 3 and above adverse reactions,the combination group was 28.00%,while the control group was 25.00%.There was no statistically significant difference between the two groups(P>0.05).Conclusion The combination of PD-1 inhibitor immunotherapy drugs and Lenvatinib targeted drugs has shown significant advantages in efficacy and prolonged survival in the treatment of unresectable hepatocellular carcinoma.It can bring better and longer survival expectations to patients without significantly increasing the risk of adverse reactions,and is worth promoting in eligible patients.
