Analysis of irrational PrescriPtions in Pharmacy intravenous admixture services of our hosPital

Abstract：Objective To analyze reasons of the irrational prescriptions that have been used in pharmacy intravenous admixture services(PIVAS)so as to promote rational drug use in our hospital. Methods The prescriptions of PIVAS in our hospital from August 2012 to May 2014 were analyzed statixtically in the article. Results Among 102 092 prescriptions, 411 prescriptions were irrational drug use,accounted for 0. 40% ,including improper solvent,unsuitable concentration,over-dose,unsuitable combination,inappropriate route of administration,incorrect input of information and lower the effective dose. Conclusion PIVAS pharmacists can improve the use of drug by correcting irrational prescriptions and reduce irra-tional drug use effctively,which can ensure the safe medication of patients and improve the rational drug use.
