The analysis of common irrational medical orders by pharmacy intravenous admixture services in our hospital

Abstract:Objective To analyse the irrational medical orders of the inpatients with venous transfusion in our hospi-tal,and provide reference for clinical rational use of drugs. Methods 4 263 irrational medical orders out of total 2 613 267 vein orders from the year 2012 to 2013 were retrospectively analyzed to intravenous drug use in our hospital. Results The unreasonable medical types mainly involved:computer entry error,improper selectiong of solution,the improper amount of solvent,incompatibility,improper frequency of use,improper dosage etc. Conclusion With the intervention of the pharma-cists in clinical medical orders,the incidence of the unreasonable medical orders decreased,which not only promoted the clinical rational use of drug but also improved the quality of pharmaceutical care.
