Application of Self-management Prescription under Shared Care Model in Patients with Type 2 Diabetes Mellitus

Abstract：Objective:To explore the effect of self-management prescription under the shared care model in patients with type 2 diabetes mellitus.Methods:A total of 150 patients with type 2 diabetes mellitus,hospitalized from March 1 to May 31,2023,were selected and divid-ed into the control group and the experimental group by random number table method,with 75 cases in each group.74 cases in the control group completed the study,and 66 cases in the experimental group completed the study.The control group received routine health education management,and the experimental group received self-management prescription intervention under the shared care model.The blood glu-cose levels including HbA1c,fasting blood glucose,2-hour postprandial blood glucose,self-management level before and after intervention(SDSCA),and readiness for hospital discharge at different times(RHDS)were compared between the two groups.Results:After the inter-vention,the HbA1c,fasting blood glucose and 2 h postprandial blood glucose in the experimental group were lower than those in the control group(P<0.05,P<0.01).After intervention,the scores of all dimensions and total score of SDSCA in the experimental group were higher than those in the control group(P<0.01).On the day of discharge,the dimension scores and total score of RHDS in the experimental group were higher than those in the control group(P<0.01).Conclusion:Self-management prescription under the shared care model can improve the readiness for hospital discharge and self-management level of diabetic patients outside the hospital,and also has a good effect on impro-ving their blood glucose and other metabolic indicators.
