Application of IPAT-Based Position Management Combined with Developmental Care in NICU Premature Infants with Extremely/Very Low Birth Weight

Abstract:Objective:To explore the application effect of position management based on infant position assessment tool(IPAT)com-bined with developmental care in premature infants with extremely/very low birth weight(E/VLBW)in neonatal intensive care unit(NICU).Methods:A total of 120 premature infants with E/VLBW admitted to NICU from January 1,2022 to May 30,2023 were enrolled,randomly divided into control group and study group,with 60 cases in each group,using a random number table method.The control group received routine nursing intervention,while the study group received IPAT-based position management combined with developmental care in the control group.The nutritional status[including total protein(TP),albumin(ALB),and prealbumin(PA)],neurological development[20 newborns neonatal behavioral neurological assessment(NABA)],physical development at birth and after intervention(including head circumference,weight,and length),rehabilitation indicators(including ventilator use time,gastric tube removal time,total enteral nutrition time,daily sleep time,NICU hospitalization time),and incidence of complications were compared between two groups before and after in-tervention.Results:After intervention,levels of TP,ALB and PA were increased in both groups(P<0.05),which were higher in study group than those of the control group(P<0.01).After intervention,the weight,length,and head circumference of both groups were higher than at birth(P<0.05),and the study group has higher scores than those of the control group(P<0.01).After intervention,NABA scores in each dimension were increased in both groups(P<0.05),which were higher in study group than control group(P<0.01).The ventilator use time,gastric tube removal time,total enteral nutrition time,daily sleep time,NICU hospitalization time in the study group were all shor-ter than those in the control group(P<0.01),and the incidence of complications was lower than that in the control group(P<0.05).Con-clusion:The combination of IPAT-based position management and developmental care has a significant nursing effect on E/VLBW in NICU,which can effectively improve nutritional status,enhance physical development and behavioral ability,shorten rehabilitation time,and reduce the occurrence of complications.
