Influence of Management Mode Based on Integration of Medical Care and Maintenance on Patients with Hypertension in Community

Abstract：Objective:To explore the influence of the management mode based on the integration of medical care and maintenance on the community hypertension patients.Methods:By convenient sampling,499 hypetension community patients were randomly divided into the observation group with 249 cases and the control group with 250 cases.The control group received routine hypertension management mode,and the observation group was given the hypertension management mode based on intergration medical care and maintenance.Situations of self-care ability and blood pressure of two groups were compared before and after the intervention.Results:After the intervention,the observation group′s self-care ability prognostic scores were signigicantly higher than that before the intervention,and the observation group′s was significantly higher than the control group′s(P<0.05).The systolic blood pressure in the two groups was significantly better than that before the intervention,and the blood pressure in the observation group was better than that before the intervention(P<0.05).Conclusion:The management mode based on integration of medical care and maintenance on patinets with hypertension in community can improve the self-care ability of patients with hypertension in the community.
