Application of the Procedural Framework of Health Problems for Patients with Alzheimer's Disease in the Continuity Care

Abstract：Objective:To explore the application effects of the procedural framework of health problems in the care of patients with Alzheimer′s disease ( AD) .Methods:A total of 88 cases of AD after discharge from April to September of 2015 were randomly divided into the control group and the observation group with 44 cases in each group .The control group received routine nursing continuation ,while the observation group received the procedure framework of health problems to deal with nursing care .The objective indexes were compared be-tween the two groups after the intervention .Results:After the intervention ,the observation group′s Barthel index ,the Western Aphasia Bat-tery(WAB),the Mini Mental State Examination(MMSE)scores and life quality scores were significantly higher than those of the control group(P<0.05).Conclusion:The framework of health problem solving process to carry out continuous care for patients with AD ,can im-prove the ability of daily life ,language function and mental rehabilitation ,improve the quality of life of patients with AD .
